Items: Name, name of company (in case of an individual, name of such individual), email address (including email address collected during the customer satisfaction survey), password, customer opinion and complaint regarding service quality, address, invoice number, customer number, opinion on the Company's service, etc. (including commments and questions, which may include the collection of video and audio information depending on the channel for submission of opinion)

Purpose of collection and use: Identity verification, personal identification, membership registration, customer management, performance of obligations under the Agreement (delivery of goods) and provision of other services, member management, confirmation and handling of complaints, delivery of notices, conduct of survey such as confirmation of customer satisfaction and preference, analysis of the survey results and improvement of services, and utilization of statistical data
  3. Period of retention and useuntil withdrawal from membership unless otherwise required by laws and regulations to preserve such information
